Individual Passport Appointment
Applying for a passport on your own? Choose an individual appointment based on whether your application will be completed before you arrive.Application Completed Before Your Visit
individual - Book 15 minutesChoose this option if you will arrive with your completed DS-11 application. Applications should be typed or completed in black ink and left unsigned.

Group Passport Appointment

Passport Photos
Just need a passport photo? Schedule a quick appointment for our passport photo service.Our team specializes in 2" × 2" passport photos, and your service includes both physical and digital copies. Passport photos are available as an add-on service for all passport applicant appointments.

About the DS-11 Application
The DS-11 Application form for a U.S. Passport is used when applying for the first time, for minors under age 16, or you do not meet the requirements for renewing your passport. Completing your application before your visit can help make your appointment quicker and easier.
If completing the DS-11 in advance:
- Complete the application using the U.S. Department of State Form Filler or fill it out in black ink.
- Print your application single-sided on standard 8.5" x 11" paper.
- Do not sign your application. You will sign the DS-11 during your appointment when instructed by a passport acceptance agent.
- Bring the completed application along with all required supporting documents and payment.
Complete the DS-11 form at travel.state.gov website.
Don't have access to a printer? Blank DS-11 applications are available at our office and can be completed during your appointment.
